Hello. This item is a compression shirt so laying flat on a table a 4XL measures 23 inches from the armpit out to the end of the cuff. It certainly stretches out to 27 or more inches easily, but the amount of stretch you get there will also depend on how your arms fill out the rest of sleeve too. \
