Hello. All of the replacement pads with the exception of the Removable Pad are designed to be sewn into a pair of snug-fitting shorts, capris, compression tights, etc. That will help to keep the pad in place best. And cycling bottoms are designed to be worn without underwear.
The Removable Pad is a little different in that it has silicone gripper material on the back side to help keep it in place, but you still would want to wear it with snug-fitting compression bottoms for best results.
